To assist us in responding to individual requirements, please inform the College of any special needs your child has which may affect his/her learning, participation or welfare during school hours.
If required, please download and complete a Medical Notification form.
If a student is required to take prescribed medication during the day, either temporarily or on-going, the parent/guardian needs to inform the College by downloading and completing a Medication Permission Form
The following applies:
- Medication must be in its original container, labelled with the student's name, medication name, dosage, and frequency of administration.
- Authority from a registered medical practitioner with the reason for the medication must be provided.
- The medication can only be administered before the expiry or use-by date.
- The parent/guardian must immediately notify any changes in medication or dosage.
- The original script or authority to be provided and/or a copy.
- The student is to come to Student Reception at the appropriate time to self administer the dose, or it must be administered by the parent/guardian.
- Webster Packs are required for all overnight camps/excursions.
Pain relief medication such as Panadol cannot be given to students by College staff. Students may not have prescribed medication with them or in their bags under any circumstances.
